HRSA just issued the “FY 2025 Quality Improvement Fund – Transitions in Care for Justice-Involved Populations (QIF-TJI)” NOFO. This program provides grants to “strengthen transitions in healthcare for persons soon to be released from incarceration” under the Second Chance Act. There’s $51M available with grants to $1 million over two years. Only FQHCs are eligible applicants, but you must act quickly, as the grants.gov deadline is June 10 and the EHBs deadline is July 2.
